Nutrition Counseling is provided to individuals who have acute condition or changes in their health status that require education in their dietary regimen to control or management of complications related to their medical conditions.
GSNY Dietician / Nutrition Counselor provides assessment and evaluation of the client's nutritional status and makes recommendations for alterations in dietary regimens based upon the prescribing practitioner's prescribed diet. The goal of nutrition counseling is to help a person make and maintain dietary changes.
Nutrition counseling services may include:

Assessment of nutritional status and food preferences
2.
Planning for the provision of appropriate dietary intake within the waiver participant's home environment and cultural considerations
3.
Nutritional education regarding therapeutic diets as part of the development of a nutritional treatment plan
4.
Regular evaluation and revision of nutritional plans
5.
The provision of in-service education to the waiver participant, family, advocates, waiver and non-waiver staff as well as consultation on specific dietary problems of the waiver participants
