Social Worker's Role in Client Care

GSNY social workers play a big role in client care. Their primary role is to help clients and their families understand an illness, come to terms with the emotions of a diagnosis, and provide counseling on the decisions. Social workers can be a family’s biggest ally, problem solver and an invaluable resource. 

 

Our goal is to improve or maintain the emotional, physical, functional and social health status of the client.

All GSNY social workers function in accordance with GSNY Home Care code of conduct and the  Standards for the Practice of Social Workers, including its accountability procedures.

Interventions provided by the Medical Social Worker include:

  • Psychosocial assessments

  • Financial-environmental assessment

  • Counseling

  • Short-term therapy

  • Client, family and community education

  • Community resource planning
